New Patients

Please call our office if you have any questions or need to reschedule your first appointment.

We do require 24-hours notice if you are unable to keep a scheduled appointment. It is our policy that after three (3) missed appointments we reserve the right to discharge you from our practice.

In order to make your first visit more effective, we ask that you come prepared with the following items:


  1.  New Patient Paperwork filled out

  2.  Referral sending you to our office by a provider

  3.  All of your health insurance cards (we will ask for them at every visit)

  4.  Photo identification (current driver’s license, employee badge or military identification card)

  5.  All medications you are currently taking, including vitamins and over the counter  medications. This will ensure your medication list is accurate and current with correct dosages and dosing instructions.

  6.  Patients with Diabetes: Glucose meters, and list of recent blood sugars

Welcome to Diabetes Alliance of Tampa Bay. We would like to take a moment to personally welcome you to our practice. It is our responsibility to deliver the best care possible to you. We provide care for patients with diabetes and endocrine issues.

Our business hours are Monday – Friday from 8:30 AM to 5:00 PM. Patients are seen by scheduled appointment only. After hours care is provided through our practice. Patients will be directed to the appropriate On Call Provider by calling our office after hours, including weekends and holidays.
